Assessment, Policy Development and Assurance in a South Jersey County: Tracing Public Health's Core Functions From 1988 to Today
Public health is easiest to understand when it is invisible: clean water, safe restaurants and childhood vaccines rarely make the news. The first module of this course asks how the field came to be organized around a small set of functions and how those functions look in a real jurisdiction. This paper traces the three core functions of public health from the 1988 report that named them, explains how the ten essential public health services grew from them and were revised in 2020, and applies both frameworks to Cumberland County, New Jersey, a mostly rural county of about 156,000 people at the southern end of the state. It closes with what the frameworks mean for the public health nurse.
Where the Core Functions Came From
By the late 1980s, many observers believed public health in the United States had lost its sense of purpose. Health departments were fragmented, underfunded and increasingly occupied with delivering clinical care to people who had nowhere else to go. A national committee answered with a report that put the field's purpose in terms of conditions, the ones under which a whole community can stay well, and it gave government health agencies three core functions to carry out (Institute of Medicine, 1988). Assessment means regularly and systematically collecting, analyzing and sharing information on the health of the community. Policy development is what an agency does with that picture, turning it into priorities, rules and plans that others can be held to. Assurance is the promise that the services a community has agreed it needs actually exist, delivered by the health department, by partners it funds or persuades, or by private parties the law compels.
The report's lasting contribution was to define public health by what it does for a population rather than by the programs it happens to run. A clinic is not public health simply because a health department operates it; it becomes public health when it is part of a deliberate response to an assessed community need. That distinction still shapes how health departments justify their budgets and how public health nurses describe their work. It also explains a tension many health officers describe: when a county runs out of other options for uninsured residents, the department often ends up running clinics, and the time and money spent there are no longer available for the population-wide work only government can do. The 1988 committee saw that drift clearly and argued that clinical services should be provided by the department only when no one else could provide them.
From Three Functions to Ten Services
In 1994 a federal working group translated the three functions into ten essential public health services, giving health departments a more detailed list of activities to plan and evaluate against. The services were revised in 2020 through a national process that placed equity at the center of the framework, so that every service is now described in terms of promoting health for all people and removing barriers that produce unequal outcomes (Public Health National Center for Innovations, 2020). The revised list keeps the three functions as its organizing structure. Assessment includes monitoring population health and investigating hazards; policy development gathers the services about informing the public, working through partnerships, and writing and defending policy and law; and assurance holds the rest, from enforcing laws and connecting people to care to staffing the field well, learning through evaluation and research, and keeping the agency itself sound.
The Functions in Cumberland County
Assessment in Cumberland County starts with data that are publicly available and sobering. Model-based PLACES estimates from federal survey data (Centers for Disease Control and Prevention, 2025) place the county highest among New Jersey's twenty-one counties on several measures in 2023, including age-adjusted adult diabetes at 12.1 percent, obesity at 40.4 percent, current smoking at 16.6 percent and food insecurity at 24.7 percent. Census estimates for 2024 put the poverty rate at 13.5 percent, compared with 9.2 percent statewide, and median household income at about $68,000, compared with about $104,000 for New Jersey (U.S. Census Bureau, 2025). A county health department's assessment function is to turn figures like these into a shared picture of community health, usually through a community health assessment completed with the local hospitals.
Policy development follows from that picture. A county with the state's highest diabetes and obesity estimates has reason to prioritize chronic disease prevention, nutrition access and tobacco control, and to build partnerships with the hospital system, the county's food bank and its agricultural employers. Assurance then asks whether services actually reach people: whether diabetes prevention classes are offered in Spanish in a county where more than a third of residents are Hispanic, whether residents without cars can reach a clinic, and whether laws on food safety, lead and communicable disease reporting are enforced. Each of the three functions is visible in the county; none is complete.
What This Means for Public Health Nursing
Public health nurses work across all three functions. The scope and standards of public health nursing define the specialty by its client, a whole population, and by the mix of nursing, social science and public health knowledge it draws on to protect and improve that population's health (American Nurses Association, 2022). In practice, a public health nurse in a county like Cumberland might collect data during home visits and immunization clinics, which serves assessment; bring what families describe to a coalition meeting, which serves policy development; and connect a newly diagnosed resident to a diabetes education program, which serves assurance.
The frameworks also give nurses language for work that is otherwise hard to explain. A visit that helps one family enroll in food assistance is individual care, but tracking how many families face the same barrier and reporting it to the health officer is public health. The core functions remind nurses that the second step is part of the job. For me, as an emergency nurse beginning this course, that move from the one patient on the stretcher to the hundreds who never reach it is the main thing the first module asks me to practice.

NUR 4430 Module 1 instructions, in plain terms
The opening module of NUR-4430 typically introduces the history and foundations of public health. The written piece for this module usually wants three moves: lay out the core functions and the essential services, sketch how they came about, and test them against a health department or community you can see from where you live. Some sections ask you to interview a public health nurse or review your local health department's website, and others focus on a historical figure or event. The prompt itself lives in the TESU classroom; read it for whether your mentor wants local data, a particular number of sources, or a reflection on your own transition to population-focused practice. Use current versions of the frameworks.
How the NUR 4430 Module 1 example is put together
The example opens by explaining why public health is easy to overlook, then sets out its plan. A history section explains the conditions that led to the 1988 report and defines each core function in plain language. The next section explains how the ten essential services grew from the functions and how the 2020 revision placed equity at the center, grouping the services under the three functions. The application section uses five current county figures from named sources to show assessment, then explains what policy development and assurance would look like in the same county. The final section applies the frameworks to public health nursing and ends with the writer's own shift from individual to population thinking.
Reading the NUR 4430 Module 1 grading rubric
A mentor reading this paper is checking five things: whether the functions and services are explained correctly, depth of application to a real community or agency, use of current and credible sources, discussion of the nursing role, and writing. Mentors give heavy weight to application, and a paper that defines the frameworks well but never applies them tends to stay in the middle band. Naming a local figure with its year and source proves application faster than any amount of description. Accuracy matters on the frameworks themselves: the essential services were revised in 2020, and papers using the older wording without noting the change may lose points. Clear headings that follow the assignment's questions help the mentor find each element.
NUR 4430 Module 1 help from the desk
The most frequent problem in the first public health paper is a history essay that never reaches the present. Give the history a paragraph or two and let the present-day application take the rest. A second trap is a ten-item list with one line per service, a format that crowds out any thinking; group them under the three functions and give examples from one place. Students also cite outdated data or national figures where local ones exist. County data from CDC PLACES and the Census Bureau are free and current. Finally, connect the frameworks to nursing practice rather than ending with the health department. A desk writer can prepare a version built on your own county and its numbers if you send your directions.

NUR 4430 Module 1 questions, answered
Where can I find a free NUR 4430 Module 1 sample paper?
It is above in full: a core functions and essential services paper for TESU's public health nursing course, with references and margin notes. The county is Cumberland, New Jersey; reuse the structure with your own county's data.
What are the three core functions of public health?
Assessment, policy development and assurance. They were named by the Institute of Medicine in 1988. Assessment gathers and shares data on community health, policy development turns data into plans and laws, and assurance makes sure needed services and protections actually reach people.
Were the ten essential public health services updated?
Yes. A national revision in 2020 kept the three core functions as the frame but rewrote the services to put equity at the center. Use the 2020 wording in NUR-4430 papers and note the change if you discuss the original 1994 list.
Where can I find health data for my county?
CDC PLACES provides model-based county estimates for dozens of measures, and the Census Bureau's American Community Survey provides income, poverty, insurance and population figures. Your state health department and the local community health assessment are also good sources.
How does a public health nurse use the core functions?
Through daily work: collecting data during visits and clinics supports assessment, sharing community needs with coalitions and officials supports policy development, and connecting people to services or enforcing health rules supports assurance. Papers that give concrete examples of each score well.
